Understanding Mydriasis: Causes, Symptoms & When To Worry

Ever noticed someone's pupils seem unusually large, even in bright light? This condition, known as mydriasis, occurs when the pupil remains dilated and fails to respond to light changes, signaling a potential underlying issue.

Mydriasis isn't simply about having big pupils. It's specifically defined as pupils that remain dilated regardless of the ambient light. Normal pupil dilation is a natural response to dim light, allowing more light to enter the eye and improve vision. Pupils also widen when focusing on distant objects, or due to the use of dilating eye drops during eye exams. However, mydriasis refers to a persistent dilation that doesn't correspond with these normal stimuli. This lack of responsiveness is what distinguishes it from typical pupil dilation and warrants further investigation.

The underlying causes of mydriasis can be diverse, ranging from relatively benign to potentially life-threatening. Certain medications, both prescription and over-the-counter, can induce mydriasis as a side effect. Recreational drugs are also known to cause pupil dilation. In other cases, mydriasis can be a sign of a more serious medical condition, such as a head injury, stroke, or brain tumor. The pressure that builds inside the brain following such events can damage the iris muscles responsible for pupil constriction and dilation.

In unconscious patients, a fixed and dilated pupil can be a critical sign of brain herniation due to an intracranial mass lesion. However, it's important to note that a fixed and dilated pupil in an awake patient is typically not indicative of herniation, suggesting a different underlying cause.

Traumatic mydriasis can also occur as a result of direct injury to the eye. For instance, a case study presented traumatic mydriasis and accommodative dysfunction secondary to a sweetgum ball ocular injury, which resolved 8 years after the initial trauma. This highlights the potential for long-term effects from eye injuries and the importance of careful monitoring and management.

The opposite of mydriasis is miosis, characterized by pinpoint pupils. While mydriasis involves excessive dilation, miosis involves excessive constriction of the pupils.
